This position directs menu planning for the skilled nursing residents, assists in the implementation of sanitary programs, and overall special diets program under the supervision of the Executive Chef.

Minimum Data Set Coordinator is responsible for the development, maintenance and implementation of the standards of nursing practice and objectives as it relates to the Minimum Data Set requirements and with the established policies and procedures of the governing body in compliance with federal, state and local regulations. This position develops a proposed plan of care in conjunction with the interdisciplinary team ensuring that the plan of care is consistent with the current health assessment for every resident under the supervision of the Director of Nursing Services.

RN assists the care plan team in the preparation, implementation and evaluation of resident assessments and care plans. This position charts for assigned units, assigns residents to nursing attendants as needed, and assists in the medication management, dietary restrictions and overall healthcare programs for the residents on an assigned unit under the supervision of the Director of Nursing Services or Charge Nurse.

The Social Worker coordinates the successful admission and transfer of all residents within the Skilled Nursing Facility and Assisted Living / Retirement Home sectors of the campus. This position assures that a social work representative is an active member of the Care Plan Committee and is responsible for the resident and the formulation of a social plan of care for him or her. This is done through provisions and services to the resident or resident family. This position oversees the development and maintenance of the social work components within the facility to meet the objectives of the skilled nursing center as a total patient care outlet, under the supervision of the Healthcare Administrator (Skilled Nursing).
